We are proud to provide our services, along with 24/7 access to our staff, to people who are facing chronic homelessness and who are at least 18 years old.

A chronically homeless person has faced repeated instances of homelessness while often living with a disabling condition such as a substance use disorder, severe mental illness, and/or disability. These conditions typically prevent them from meeting the eligibility requirements at traditional shelters.

Temporary Housing

We provide safe housing for adults experiencing chronic homelessness through our Motels4Now (M4N) program. Guests are placed in a single or double-occupancy room with a bathroom. Bed / bath accessories and products are also provided.

At 30 years old, Kadeem was faced with the choice of losing his job at Kroger or going to the hospital to treat his frostbite. He chose work. By the time he made it to the hospital, doctors had to remove all ten of his toes to save his life.

Wraparound Support

Guests also have access to onsite services to support a range of complex needs that chronically homeless individuals often face. These include addiction and mental health disorders, food insecurity, lack of access to healthcare, transportation barriers, and disabilities.


Mental Health & Addiction Services

Guests are connected with an on-site psychiatrist who diagnoses and treats various mental health conditions, including substance use disorders. Staff, along with our mental health partner, Oaklawn, lead guests in conversations about addiction recovery and offer additional mental health support.

Food Program

In partnership with Our Lady of the Road, we serve 200+ meals (lunch and dinner) daily to guests. We also provide snacks, coffee, breakfast items, and other pantry items.

Access to Healthcare

A team of physicians meets with guests on-site to treat acute illnesses and coordinate appointments with primary care doctors.

In 2022, the Street Medicine Clinic was formed in partnership with Beacon Health System to increase access and quality of healthcare, while decreasing overall healthcare costs.

Transportation

Guests have access to free bus passes and daily transportation for medical needs, addiction treatment and legal appointments.

Long-Term
Housing Support

Our goal is to help every guest successfully transition to long-term, stable housing or permanent supportive housing (PSH). Our Housing Team partners with guests in the following ways to assist them before, during, and after their transition:

  • Secure ID documentation and rental history.

  • Complete applications for financial support programs (vouchers, social security, veterans benefits, etc.) and permanent supportive housing. 

  • Partner with local landlords to identify suitable apartments.

  • Provide essential furniture and housewares.

  • Visit with former guests monthly to assist with potential concerns and needs such as utility payments, transportation, and neighbor or landlord relations.
